I am both the employer and service user. I have a diagnosis of Border line personality disorder, anorexia nervosa, Complex PTSD and suffer from anxiety and depression. I have at times been known to have self-harming behaviors. Due to this I am extremely isolated, spending significant time at home with only the support of family members. I am seeking a PA to assist and support me with my independence and an ability to access the wider community, in social activities, keeping me occupied and motivated. The PA role will be to provide friendship, companionship, and support, helping me in building my confidence, encouraging me to try new things and experiences away from the family home. I require support around mealtimes, with distractions, as this can provoke times of higher levels of anxiety for me. I enjoy a range of activities but require the support of others, I particularly enjoy going swimming, cold dips, going to the gym, being active going for walks, being outdoors, going to the cinema, swimming, having my nails and hair done and generally being out in the community and socialising with others. The PA will also be required to assist and support me at appointments, provide prompts and reminders of these and at times spend time with me in my home. Assistance and prompt with cleaning and general housework The PA support will also allow my parents a break from their caring role and enable me to develop my social support network • To be proactive in identifying suitable activities and venues and be willing to get involved in my choices. • The role is to befriend and offer companionship with a view to extending my social and support networks. • To promote and enable me with independence and the opportunity of time with someone other than a family member. • Accessing activities • Occasionally providing support and engaging in activities within the family home • Promoting and encouraging me to engage with others in a social setting • To support me going to and at appointments
